The Structural Pest Division enforces compliance with the Georgia Structural Pest Control Act and rules by routinely inspecting structural pest control work, investigating complaints and performing administrative duties related to examinations and the issuance of company licenses, certified operator cards and registered employee cards.
To request an inspection, you must complete this form explaining your problem and the desire for a state inspection. You will need to include your name, address, and phone number where you can be reached, and the name and location of the company involved. You should gather all contractual agreements or other documents related to the incident/ application and upload copies when completing the form or provide copies to the inspector. The inspector will need these copies to include in the investigation file.
